The following information is for use by CCHP health care professionals and facilities only.

Claims Status Inquiry

NEW CCHP Provider Portal

Login or create an account to view and maintain your CCHP Provider Portal account. From this site you'll be able to:

  • View patient's health plan benefits and summaries
  • Find a network healthcare provider
  • View patient's healthcare claims and deductible/out of pocket balances

Web Based Inquiry (Epower will no longer be available 1/1/2018. Please start using the NEW CCHP Provider Portal immediately.)

CCHP contracted health care professionals and facilities can check the status of their claims using the CCHP "Web Based Inquiry" portal. Please click on the above "Web Based Inquiry" link to log-in with your UserID and Password. After checking online, if you have a question about a claim, please call Member Services Center at 1-415-834-2118.

If you need assistance with web access, please call Provider Relations at 1-415-955-8800, ext. 3372, Monday through Friday, 9:00 AM to 5:00 PM.

If you are not a contracted provider with CCHP or do not have web access and would like to check claims status, please call Member Services Center at 1-415-834-2118.

Submitting Claims Electronically

CCHP prefers claims be submitted electronically. For information, please call our Member Service Center at 1-415-834-2118.

Submitting Paper Claims

All paper claims must be submitted using a CMS 1500 form (for professional providers) and a UB-92 form (for institutional providers).
