We offer Medicare Advantage Prescription Drug (MAPD) plans in San Francisco and San Mateo counties, and as of plan year 2025, Alameda County as well.
Founded in 1986 in San Francisco as an Original, our mission is to improve the health of the diverse communities we serve. We offer plans with and without premiums and generous flex allowances to help you buy healthy groceries and Over-The-Counter (OTC) items, transportation to medical appointments, and worldwide ER coverage. We also have programs designed to encourage and improve the health of members who require extra attention for chronic conditions.

CCHP is a recipient of a Bronze Award for the prevention of heart attacks, strokes and diabetic complications by California Department of Managed Health Care (DMHC).
Together with Chinese Hospital, CCHP was awarded the National Council of Asian Pacific Islander Physicians (NCAPIP) Inaugural Excellence in Health Award for being pioneers in providing access to quality and affordable care for the Chinese American community in San Francisco for over one hundred years.
