¹ CCHP Senior Value Program (HMO) and Senior Select Program (HMO).

² Up to $940/year in combined OTC and grocery purchases with CCHP Senior Select Program (HMO D-SNP). $15 monthly OTC allowance is automatic and a one time $100 grocery allowance is qualified upon completing the Annual Wellness visit. Another $45 monthly grocery allowance for qualified members with chronic conditions plus, a $10 monthly bonus is available for hypertensive members meeting certain requirements.
