From March to July, you may receive up to two surveys from Medicare – CAHPS survey (Consumer Assessment of Healthcare Providers and Systems), and HOS survey (Medicare Health Outcomes Survey).
The Consumer Assessment of Healthcare Providers and Systems (CAHPS) Survey collects information on your experience and satisfaction with the health care you receive from your health plan and providers.
The Medicare Health Outcomes Survey measures the quality of care you receive from your health plan and providers by asking questions about your health over time.
The paper survey will be mailed to you by a vendor called SPH Analytics. If you do not return your completed survey, SPH Analytics may contact you by telephone to help complete the survey.
