CCHP Will Continue Accepting New D-SNP Enrollees Through 2026

Many San Francisco residents who rely on health plans that coordinate their Medicare and Medi-Cal (Medi-Medi) benefits have recently been concerned about losing access to coverage. Beginning in 2025, changes to state rules limited which health plans can accept new members who qualify for both Medicare and Medi-Cal. These changes caused confusion for many San Francisco residents who rely on this type of coordinated coverage.

CCHP has been continuously served the Medi-Medi population in San Francisco with its Dual Eligible Special Needs Plan (D-SNP), known as the Senior Select Plan, for over 20 years. By working with governmental agencies such as Centers for Medicare and Medicaid Services (CMS) and Department of Health Care Services (DHCS), they see the value of this partnership. CCHP is reassuring the community that will remain open to new enrollments through 2026. This ensures that older adults and individuals with complex health needs can continue to receive the care and services they rely on. This plan is only available to San Francisco residents and is designed to help members manage their medical benefits and access essential services like dental, vision, hearing, transportation assistance, and allowances for groceries or over-the-counter health products.

With multilingual staff and physical Member Services locations, residents can walk in and receive personal assistance with eligibility, enrollment, and benefits — support that is especially valuable for seniors and new immigrants who may find the healthcare system difficult to navigate.

Chinese Community Health Plan (CCHP) is a Medicare Advantage HMO plan with a Medicare contract and a California Medicaid program contract for our SNP. Enrollment in CCHP depends on contract renewal.
