- No changes are required if you’re already in a Medi-Medi plan. These Dual Eligible or D-SNP plans—including CCHP’s Senior Select Program (HMO D-SNP)— will continue to serve your health care needs without interruption.
- Individuals who have Medicare and Medicaid (Medi-Cal) can enroll into CCHP Senior Select Program (HMO D-SNP) under the following conditions:
- Before March 31: Please reach out to one of our CCHP certified representative to assess if you qualify to enroll into CCHP Senior Select Program (HMO D-SNP).
- April 1 to December 31 (Special Enrollment Period – SEP): individuals who experience certain qualifying life events can enroll into the CCHP Senior Select Program (HMO D-SNP) within 60 days from the qualifying event. Please reach out to a CCHP certified representative for additional information.
