From March to July, you may receive the CAHPS survey, which is done yearly. Please complete the survey as your feedback will help us improve our services. The Consumer Assessment of Healthcare Providers and Systems (CAHPS) Survey collects information from your experience and satisfaction with the health care services you received from your health plan and providers in the last 6 months.
The survey will be mailed to you by our vendor SPH in a white envelope. A return envelope will be provided for you. If the survey is not returned, SPH may contact you by telephone to help complete the survey.
