As of June 30, 2025, CCHP is no longer opting in new members into the Hypertension Management Program.
This program was designed for the CCHP Medicare Advantage Program members who have been diagnosed with high blood pressure, also called hypertension. It helps participants monitor their blood pressure in the comfort of their home.
