When can you enroll?
Senior Program (HMO) & Senior Value Program (HMO)
Medicare Annual Enrollment Period is Oct. 15 through Dec. 7.
For people turning 65 years of age:
3-months before and 3-months after your 65th birthday month.
Senior Select Program (HMO D-SNP)
Medicare annual enrollment period is 10/15 – 12/7. For people turning 65 years of age, you can enroll 3 months before and 3 months after your 65th birthday. You can also enroll during a Special Election Period which occurs once per calendar quarter during the first 9 months of the year.

Chinese Community Health Plan (CCHP) is a Medicare Advantage HMO plan with a Medicare contract and a California Medicaid program contract for our D-SNP Plan.
