Medication Therapy Management (MTM) Program
This service is designed for members with multiple health conditions and for those who take multiple medications. MTM ensures you and your doctor that your medicine is effective.
CCHP Pharmacists are available to discuss your medication.
In partnership with OutcomesMTM™, we extend MTM services to you through specially trained OutcomesMTM Pharmacists at pharmacies across San Francisco and San Mateo counties.
Our role extends beyond drug dispensing.
We are there to help you optimize your medication’s outcomes while keeping out-of-pocket costs down. By utilizing the free services of a CCHP Pharmacist or OutcomesMTM Pharmacist, you gain the additional attention necessary to ensure your medications stay on the right track.
Who Is Eligible?
To qualify for the CCHP MTM program, you must meet the following 3 criteria:
- You must have 3 or more chronic diseases:
- Diabetes
- Dyslipidemia (High Cholesterol)
- Respiratory Disease (Asthma, Chronic Obstructive Pulmonary Disease (COPD), and other chronic lung disorders)
- Bone Disease-Arthritis (Osteoporosis, Osteoarthritis, Rheumatoid Arthritis)
- Chronic Heart Failure
- Alzheimer’s Disease
- End-stage renal disease (ESRD)
- Human immunodeficiency virus/Acquired immunodeficiency syndrome (HIV/AIDS)
- Hypertension
- Mental health (including depression, schizophrenia, bipolar disorder, and other chronic/disabling mental health conditions)
- You must take 8 or more chronic medications
- You must have a total drug cost of over $1,276 a year on medications
* At-Risk-Beneficiaries (beneficiaries who may potentially misuse or overdose on opioids) will be enrolled into our MTM program
How Can It Help You?
If you qualify for the MTM Program, you will have the opportunity to meet one-on-one with a pharmacist to discuss and review your medication(s) at no charge to you.
The pharmacist will:
- Assess your health history including conditions, prescription medications, over-the-counter drugs and herbal supplements;
- Talk about side effects that may make it hard to take your medicine;
- Suggest the best ways to take your medicine;
- Suggest other medications that may work for you;
- Answer any questions you may have regarding your medications.
What Is Included?
Your participation in the MTM Program includes these services:
- Comprehensive Medication Review (CMR) – Estimated Time: 30 minutes
Your CCHP Pharmacist or OutcomesMTM Pharmacist will meet with you face-to-face or via phone to review all your medications. This will help identify any potential medication problems and help organize your medication schedule. After the CMR, you will receive a Medication Action Plan and Personal Medication List that you can share with your doctor or caregiver. - Prescriber Consultation – Estimated Time: 5 to 15 minutes
Your CCHP Pharmacist or OutcomesMTM Pharmacist will discuss with you and your doctor(s), face-to-face or via phone to resolve any problems found with your medications. - Medication Adherence Checks – Estimated Time: 5 to 15 minutes
Your CCHP Pharmacist or OutcomesMTM Pharmacist will ask about how you take certain medications to ensure you take it as prescribed and work with you on any barriers preventing you from taking your medications regularly.
How Will You Be Notified?
Members that qualify for MTM will be contacted by CCHP through the mail. Members may also be contacted by their CCHP Pharmacist or OutcomesMTM Pharmacist face-to-face or via phone.
- Members eligible for MTM will be identified by CCHP at least four times a year through a review of their medication history.
- Eligible members are able to opt out of the MTM program as participation is voluntary. Contact Member Services if you would like to opt-out of the MTM program.

- Find a CCHP Pharmacist or OutcomesMTM Pharmacist by contacting Member Services. Ask for the locations of a CCHP Pharmacist and OutcomesMTM Pharmacist in your area.
- Not all pharmacists in the CCHP pharmacy network have finished a special training course by OutcomesMTM to provide these extra services.
- You may use your regular pharmacy as well as gain access to a CCHP Pharmacist or OutcomesMTM Pharmacist, which is an added value of your CCHP membership. You may continue to use any CCHP participating pharmacy for your prescriptions.
- There are no additional costs for the use of CCHP Pharmacists or OutcomesMTM Pharmacists.
CCHP Pharmacists and OutcomesMTM Pharmacists are available to all members, but those with diabetes, high blood pressure, or other conditions treated with medications may see greater benefit. - You will save money using CCHP Pharmacists and OutcomesMTM Pharmacists. Similar medications can have large cost differences. Your Pharmacist can help you take the least costly, most highly effective medications.
- Your doctor will know if any changes need to be made to your prescriptions. Your CCHP Pharmacist or OutcomesMTM Pharmacist may make recommendations to you and your doctor(s), but only your doctor can change your prescription. Pharmacists will not be able to change your prescription without your doctor’s approval.
- Access to a CCHP Pharmacist and OutcomesMTM Pharmacist is an added service of membership. Although they are there to help, you are not required to use them.
- To get started, contact a CCHP Pharmacist or a nearby OutcomesMTM Pharmacist to schedule your Comprehensive Medication Review (CMR). Budget about thirty (30) minutes to review your medication with the Pharmacist.
- If you have trouble getting to a pharmacy, Chinese Hospital, or CMR location, we can arrange a MTM via telephone. Contact a CCHP Pharmacist or a nearby OutcomesMTM Pharmacist to request a telephonic CMR.
How to Dispose of Unused or Expired Medicine
Get rid of unused or expired medicine as soon as possible. Read the information that came with your medicine. It might tell you how to safely get rid of it. If you don’t have the information, follow one of these safe options:
1. Ask your local pharmacy if they have a program to get rid of medicine you do not need anymore.
- Some pharmacies (and other DEA approved sites) allow medicine to be mailed to the pharmacy.
- Ask for the special packages needed to mail medicine.
2. Bring the medicine to a community Drug Take Back program. This is the best method for controlled substances.

3. Find a DEA Year-Round Drop-off Location
4. Follow the directions of the local and national resources below. Do not throw away medicine in the trash or flush approved medications.
