From March to July, you may receive the CAHPS survey, an annual survey designed to monitor and improve the quality of care provided to members like you.
The Consumer Assessment of Healthcare Providers and Systems (CAHPS) Survey collects information based on your experience and satisfaction with the health care services you received from your health plan and providers.
The paper survey will be mailed to you by a vendor called SPH Analytics. If you do not return your completed survey, SPH Analytics may contact you by telephone to help complete the survey.
Please complete and return the survey before their cut-off date, so we will be able to improve our services and your experiences. All provided information will be kept confidential, and it will not affect the care you receive. For any translation help, please contact CCHP Member Services.
