Chinese Community Health Plan (CCHP) is a Medicare Advantage HMO plan with a Medicare contract and a California Medicaid program contract for our SNP. Enrollment in CCHP depends on contract renewal. Medicare beneficiaries may also enroll in Chinese Community Health Plan through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.
