In the course of providing the health benefit programs, CCHP must collect, use and disclose protected health information. CCHP considers this information confidential and private and consequently, CCHP has policies and procedures in place to protect the information against unlawful use and disclosure. In addition, CCHP is required by law to:
- Keep confidential your protected health information while providing for the uses and disclosures described here which are necessary to administer your health care benefits;
- Provide you with this Notice of our legal duties and privacy practices pertaining to your protected health information; and

4. Privacy and Disclosure Statements
CCHP makes the following assurances of privacy for visitors to the website:
- The website is designed to provide useful information about the CCHP organization and about health.
- The website does not contain any advertising for products by third parties, nor does CCHP share any of your information with other parties except as authorized by law.
- CCHP tracks visits to the website via an automatic monitoring program which records, among other things: how many visits are made to the website; the time of day and date of those visits; and the areas visited. This information is used to evaluate the effectiveness of the website. It helps CCHP answer questions such as: is CCHP providing information on the website that is useful? which information is most useful? and how has CCHP made you aware that the website exists? The monitoring program does not provide CCHP with any personal information about the visitor. CCHP cannot discern the name, address or any other personal information about visitors to the website.

6. Who Funds the Website
The website is funded entirely by the operating income of the organization. CCHP does not accept external advertising income in exchange for promotion of any product or service via the website.
8. Liability of CCHP
The use of the website and Content is at your own risk. When using the website, information will be transmitted over a medium that may be beyond the control and jurisdiction of CCHP and its suppliers. Accordingly, CCHP assumes no liability for or relating to the delay, failure, interruption, or corruption of any data or other information transmitted in connection with use of the website.

You expressly agree that exclusive jurisdiction for any dispute with CCHP, or in any way relating to your use of the website, resides in the courts of the State of California and you further agree and expressly consent to the exercise of personal jurisdiction in the courts of the State of California in connection with any such dispute including any claim involving CCHP or its affiliates, subsidiaries, employees, contractors, officers, directors, telecommunication providers, and content providers.
The terms and conditions of use are governed by the internal substantive laws of the State of California, without respect to its conflict of laws principles. If any provision of the terms and conditions of use are found to be invalid by any court having competent jurisdiction, the invalidity of such provision shall not affect the validity of the remaining provisions of the terms and conditions of use, which shall remain in full force and effect. No waiver of any of the terms and conditions of use shall be deemed a further or continuing waiver of such term or condition of use or any other terms or conditions.

Chinese Community Health Plan (CCHP) is a Medicare Advantage HMO plan with a Medicare contract and a California Medicaid program contract for our SNP. Enrollment in CCHP depends on contract renewal. Every year, Medicare evaluates plans based on a 5-star rating system. CCHP’s pharmacy network offers limited access to pharmacies with preferred cost sharing in San Francisco and San Mateo County. The lower costs advertised in our plan materials for these pharmacies may not be available at the pharmacy you use.
