The Medical Social Worker works in conjunction with Care Coordination staff and interdisciplinary team consisting of the medical director, utilization management staff and others involved in the members care in order to meet the global needs of the member and their family. Interventions may include assessing the psychosocial functioning of the member and family, assistance with discharge planning, connecting members and families to necessary resources and supports in the community; arranging psychotherapy, supportive counseling, or grief counseling; or helping a member to expand and strengthen their network of social supports.
The Medical Social Worker must work cooperatively with other members of the multidisciplinary treatment team who are directly involved in the member’s care. Excellent analytical and assessment skills, an ability to communicate clearly with both members and staff, and an ability to quickly and effectively establish a therapeutic relationship with members is essential to this position. The medical social workers must be willing to act as advocates for the members, especially in situations where the medical social worker has identified problems that may compromise the member’s health and safety.
